This is important as it is necessary to establish where and when your injury occurred. This may be difficult if you do not report the incident as soon as possible after the accident. Your employer should record your injury in your work place register.
You will need to see your nominated doctor as soon as possible after the accident in order that your doctor can confirm you have suffered an injury and record the nature of the injury. If you do not seek medical attention as soon as possible after the accident, it may be difficult to establish at a later date when the injury occurred. Your doctor will also need to provide you with a Workcover Certificate of Capacity.
If you need to apply for Workers Compensation, your employer should provide you with a Workers Compensation Claim Form. Once you have competed the Claim Form you need to lodge it on the relevant Workers Compensation insurer. Your employer should provide you with information about who the relevant Workers Compensation insurer is and should lodge the Claim Form on your behalf within 7 days after receiving the completed Claim Form from you.
